We have created a Contact page Template to use for your Contact Page. To enable it just select it from templates: "Contact Page Template".
This template includes a Google Map at top. To enable it enter your GPS coordinates in WP Aadmin -> Sweetdate -> Contact info & Social.

Sweetdate is ready for Contact Form 7.
If you want to quickly build your contact page, install the plugin form the following address: http://wordpress.org/extend/plugins/contact-form-7/
After you have successfully installed the plugin go to WP Admin -> Contact.
Edit contact details and fields.
Copy the shorcode for the form(example: [contact-form-7 id="91" title="Contact form 1"]) to your contact page:
